School Records/Student Transcript Request
STUDENTS WHO GRADUATED AFTER 1990 SHOULD CONTACT THE LAST SCHOOL ATTENDED. THIS OFFICE DOES NOT MAINTAIN COPIES OF DIPLOMAS; CONTACT THE SCHOOL OF GRADUATION FOR INFORMATION REGARDING DIPLOMAS.
Current or former students requiring permanent school records verification of all years in attendance, should contact the last high school attended. Click here for a school listing.
Records Management processes transcripts for former students who last attended Miami-Dade County Public Schools in 1990 or before. Individuals requesting records must complete the Student Transcript Request form (FM-7335).
Requests by MAIL must be addressed to Miami-Dade County Public Schools, Records Management Department, 2740 Northwest 104th Court, Miami, FL 33172.
Requests by FAX must be sent to 305-717-3485 or by EMAIL to firstname.lastname@example.org.
The Records Management staff replies to all requests as quickly as possible. Requests are processed on a first-come, first served basis. Requests will be processed within 20 business days after receipt of the completed form at no charge.
All requests for verification of GED test scores and/or official transcripts are processed by the Florida Department of Education, GED Testing Office.
