The Miami-Dade County Public Schools Records & Forms Management Office oversees school records for inactive students, Retention/ Disposition of documents, and Forms Management for the District.  Categories of student records/ transcripts requests have been provided below for your convenience.

Student Record / Transcript Request

Former Miami Dade County Public Schools student who attended school prior to 1990.

Complete Student Records / Transcript Request Form (FM-7335), and submit to Miami Dade County Public Schools Records & Forms Management Department, in one of the following options:

** Self-addressed stamped envelopes are to be included with the request.

Students who last attended school after 1990. Contact the last school of attendance directly to request copies of student records, transcripts, diplomas, etc.

GED (High School Equivalency Diploma)

Student requests for GED test scores/ or transcripts.

Contract the Florida Department of Education, GED Testing Office, at 1-850-245-0449.

Closed Private School Records

Select private schools, upon closure, return student records to the local school District. Access list of closed private schools that submitted student records to this District.

Complete Student Records/ Transcript Request Form (FM-7335), and submit to Miami Dade County Public Schools Records & Forms Management Department.

The Student Records/ Transcript Request Form is to be completed, signed, and submitted with a copy of a valid photo identification for processing in the order received.   For additional questions, you may email Staff at Records@dadeschools.net
